PLEASE READTo enroll in this class, please
download the application packet by clicking below. This packet
contains six pages.
The
first page is an informational flyer regarding the classes,
which includes our contact information. The
second page is the application; please circle Level 1, 2, or 3 at the top of
page. The third
page is the Rules and Regulations form. This form must be signed and dated and
returned with the other forms and payment. The fourth page is the Vet Verification
form. This form MUST be completed or the dog will not be allowed to participate in
class. Most vets are happy to complete these forms. Your information will be
verified by the instructor. This is to maintain a healthy environment for all dogs
attending classes, and to make sure all dogs are physically and mentally
capable of completing obedience classes.
All three of the forms pages 2-4 need to be
completed entirely and mailed, along with your check for $75 per dog for
Level 1 or 2 and $85 per dog for Level 3 classes,
made payable to
Suzanne Vening to the address listed on the
application
on or before the deadline listed above in order to be
considered for enrollment in Southern Charm School for Dogs classes.
If you make your check out to Southern Charm
School for Dogs, your application will be incomplete and you will be not be enrolled in class. Thank
you for your assistance with this.
Level 2 and 3 class enrollment has to be pre-approved by instructor.
If you are not sure which class you should enroll your dog in, please email
us.
Page 4, the vet verification form, MUST be completed by your vet
in order to secure your place in the class. Receipts from your vet
will not be accepted as they do not verify the results of the fecal, nor do
they verify that the dog is physically and mentally ready for an obedience
class. IMPORTANT!! If you pet had a negative fecal, you will have to have your pet retested before the pet can attend class. Bordetella is required TWICE a year (every 6 months) for these classes. IMPORTANT!!
The fifth
page includes a list of things you need to know and to bring to
class. The sixth (last page) is a goals sheet.
Please think about what your goals for you and your dogs, then use the goal
sheet to write them
down and bring to the first night of class.
